The form is used to identify who will look after a minor baby or a mentally incompetent adult. But it is not its only application. They are also used in cases where a parent is no longer able to care for their child. If a military service member on active duty was sent abroad, they might use a temporary guardianship agreement to give someone else temporary custody of their child while they were gone.
